Falls among the elderly are the leading cause of injury, emergency department visits, long term hospitalization, and even death for persons age 65 and older.  In an effort to serve Suffolk’s elderly residents covered by Medicare, the Suffolk County Department of Health, Bureau of Public Health Nursing is expanding our services to provide fall and injury prevention care services for elderly patients in the home care setting.  These services include the following:

 

  • Skilled nursing assessment
  • Physical therapy
  • Occupational therapy

This exciting new initiative is the latest component of a successful Suffolk County Department of Health falls Prevention Program. This program includes on a broad base of expertise and now includes home-based services for the elderly who have suffered a fall, or are at risk for falling.
