Our Mission

The Bureau of Public Health Nursing is committed to improving the health and quality of life for the residents of Suffolk County with an emphasis on preventive care. The mission of the bureau is aligned with the Suffolk County Department of Health Services’ mission to prevent disease, protect public health and achieve high quality equitable and affordable health services for Suffolk County’s most vulnerable populations.

Public Health Nursing

Programs

Prenatal/Postpartum Care

Public Health Nurses provide case management for high risk pregnant woman to aid in reducing infant mortality and the number of premature births. Post- Partum visits focus on women who are identified as “at risk” of having health and/or parenting problems following the birth of their infants.

Prenatal/Postpartum Care Problems identified include:

  • Gestational Diabetes Management
  • Substance Abuse
  • Breast feeding
  • Postpartum Depression
  • Uterine and C-Section infections
  • Post-delivery hypertension and hyperglycemia
  • Women needing support in caring for their newborn
  • Health assessment of well and compromised newborns

Child Find

This service can help identify a child’s problems early, and help a family obtain the services they may need. The nurse will ensure that the baby keeps doctor visits as well as performing periodic assessment and developmental screening tests.


Child Find Eligibility

Below are some examples of Child Find Criteria:

  • Infants born after a pregnancy of less than 33 weeks.
  • Infants who spend 10 or more days in a neonatal or special care unit.
  • Young children who have problems with muscle tone.

Inborn Metabolic Screening Follow-Up

New York State Department of Health is mandated to follow-up on inaccurate, abnormal or missing metabolic screening test results. The Bureau of Public Health Nursing is available to work with providers to follow up on this mandatory screening


Chronic Disease Management

The home environment can influence self-care of a patient. Lack of education and compliance also affects this process. Home visits provide comprehensive teaching, valuable education and skill development to manage your patient’s disease.

Child Abuse/Neglect

Referrals are made to Public Health Nurses for at risk families needing assistance in complying with all the health care needs of their child.


Wound Care

Visits are made to help provide patients with assessments, education, and guidelines to care for their wound at home.


For Providers

Bureau of Public Health Nursing Provides

  • Coordination with health care providers to keep them updated on status of patient
  • Assessment of home environment’s impact on patient’s overall health.
  • Improvement to Provider follow up/appointment compliance
  • Health Education and Guidance
  • Reinforcement of patient’s plan of care
  • Tracking of signs and symptoms of patient’s progress
  • Identification of real or potential obstacles for the promotion of optimal health and wellness

For Families

The Suffolk County Bureau of Public Health Nursing offers in-home nursing and rehabilitative services to residents of Suffolk County who have health needs and required home visits.

  • Nurses show caregivers how to best care for infants and children. Families will learn about proper nutrition, safety, and a child’s developmental milestones.
  • Pregnant women can also receive home visits. The nurse can help the woman understand how the baby grows and develops before birth, how to eat right during pregnancy, how best to prevent a premature birth and what to expect during labour and delivery.
  • The public health nurse can help a family obtain other services in the community that they might need.
  • Education and encouragement is provided regarding chronic disease management of a patient’s health at home in order to avoid readmission to the hospital and alleviate family concerns.
  • Helping families and patients understand their medications.

About Us

The Bureau of Public Health Nursing is a New York State Department of Health, Certified Home Health Agency working together with community programs and providers improving the health and quality of life for the residents of Suffolk County with many health needs. SBIRT and NARCAN trainings have been completed by the Public Health Nursing team. Both trainings are related to substance abuse. SBIRT is a screening process tool. NARCAN training teaches when and how to administer an aerosol form of the drug naloxone to an individual who is overdosing.

Through our many years of service, the bureau recognized the demand for maternal child health care creating our unique area of expertise. The Bureau’s mission is to promote optimal overall health and wellness strengthening the welfare of minority communities. Our unique focus is maternal child health.
