Medical Assistance (MEDICAID) provides essential medical services for persons receiving Public Assistance and other low-income persons. Services covered include inpatient and out patient hospital care, physician services, skilled nursing care, laboratory and X-ray services, early and periodic screening of children, home health care services and transportation services.

Applying for Medicaid - Helpful Information
Persons who already have an open Medicaid (not Chronic Care) case can call Undercare with any questions. The number of the Undercare Unit in the Riverhead Center (Medicaid East) is 631-852-3570. The number of the Undercare Unit in the Smithtown Center (Medicaid West) is 631-854-0555.
Persons with a Medicaid card can call 1-800-541-2831 to find out the names of primary care physicians and specialists who accept Medicaid.
Persons with an active case and medical need and with no means of transportation may get help with transportation to and from health care providers (doctor, clinic, hospital, etc.). All non-emergency trips require prior approval. Call toll free 1-866-952-1564 (24 Hours A Day/7 Days a Week) for more information.
Medicaid recipients who provide their own transport, either by private vehicle or temporary transportation, should call toll free 1-866-952-1564 (24 Hours A Day/7 Days a Week) for information on reimbursement.
Persons enrolled in the Affinity, Fidelis, Healthfirst or United Healthcare Managed Care plans must call their plan to receive help with transportation.
Individuals with both Medicare and Medicaid, who are 21 years of age or older, seeking information on various Medicaid home care programs should call: New York Medicaid Choice at: 1-888-401-MLTC (6582). Other individuals not enrolled in Medicaid Managed Care seeking information regarding non-institutional care such as personal care aide services should call the Home Care Intake Unit at 631-854-9585.

SSI recipients needing Medicaid information should call 631-853-8926. Persons who wish to "spend down" or pay an overage to qualify for Medicaid coverage, call 631-854-5879.
All Temporary Assistance, Medicaid-only and SSI-Medicaid clients may choose from six managed care plans to receive the complete menu of Medicaid benefits. There are no costs, no caps, and no co-payments. Each member has his/her own physician or pediatrician-OB/GYN, who will coordinate all their health needs. Call New York Medicaid Choice at 1-800-505-5678 to receive a packet of informative brochures on each plan and an enrollment form.
