Information for ACTIVE SNAP Cases

Please be advised that, effective February 1, 2012:

  • We will be centralizing all active SNAP and Medicaid cases for SSI recipients to the Smithtown Center, located at:

    200 Wireless Blvd.
    Hauppauge, NY 11788

This should NOT cause a hardship for you since you are NOT required to appear at a DSS Center to receive SNAP benefits. You can conduct your business with this Department by mail and/or telephone, as described below.

  • SNAP re-certification applications, required documentation, correspondence informing DSS of case changes, etc. must be mailed to:

Suffolk County Department of Social Services
    MANIT Unit
    PO Box 18100
    Hauppauge, NY 11787

If you need to speak with someone about your SSI SNAP or SSI Medicaid case, call 631-853-8926 or, 631-853-8750 or, 631-853-8861

If you need a replacement SNAP (EBT) Card, you can call the number above to request one and it will promptly be mailed to you. In an emergency, you can go to the Coram, Riverhead, Smithtown or South West Center to pick up a replacement card.

Please be assured that DSS will make every effort to continue serving our clients without interruption as we undergo this change.

Thank you for your cooperation and patience.
