Goals And Objectives Comprehensive and Continual Care The care we provide shall: Focus on preventive medicine in order to discourage avoidable and more costly acute care. Be comprehensive in terms of breadth of services available. If specialty care is required, such referrals will be closely monitored and integrated into the management of the patient’s health. Recognize that a patient’s care must extend into their homes and communities. We shall recognize that there are social, fiscal, and cultural aspects that affect a patient’s health. Couple primary and specialty care with specific disease management programs. These programs will work in close conjunction with patient’s primary care provider. Be presented to the patient with the highest level of respect, confidentiality, dignity, and compassion. Divisions Bureau of Chest Diseases The mission of the Suffolk County Department of Health Services, Division of Patient Care, Bureau of Chest Diseases (BCD) is to prevent, control and eliminate tuberculosis (TB) in SuffolkCounty. The BCD will accomplish this mission through policy development, provider consultation, technical assistance, education, supervision, regulation and surveillance.
