Medical Assistance (MEDICAID) provides essential medical services for persons receiving Public Assistance and other low-income persons. Services covered include inpatient and out patient hospital care, physician services, skilled nursing care, laboratory and X-ray services, early and periodic screening of children, home health care services and transportation services.
APPLYING FOR MEDICAID: FOR PERSONS REQUIRING CARE IN A NURSING HOME OR LONG TERM HOME HEALTH CARE
Persons who require skilled Nursing Home care, intermediate care in an institution, or Long Term Home Health Care in the community must call (631) 854-5866 or (631) 854-5895 between 8:00 a.m. and 2:00 p.m. to schedule an appointment. Appointments are conducted at the Mary Gordon Building.

Medicaid interviews for all other persons are conducted in either the Riverhead or Smithtown Social Services Center. See the Medicaid Site Zip Code Search for the center that serves your area. Persons may bring completed applications to the appropriate site Monday - Thursday at 8:00 a.m. to apply. Interviews are scheduled Monday through Thursday by appointment, and when possible walk-in interviews are conducted. Applicants who cannot be interviewed on the day their application is submitted will be given an interview appointment for the next available day. Persons unable to access their Medicaid Center due to a hardship may make special arrangements by calling their Medicaid Center.

PERSONS WITH ONGOING MEDICAID ONLY CASE
Persons who already have an open Medicaid (not Chronic Care) case can call Undercare with any questions. The number of the Undercare Unit in the Riverhead Center (Medicaid East) is (631) 852-3570. The numbers of the Undercare Unit in the Smithtown Center (Medicaid West) are (631) 853-8754, 8765, or 8823.

Persons with a Medicaid card can call 1(800) 541-2831 to find out the names of primary care physicians and specialists who accept Medicaid.
MEDICAID TRANSPORTATION: (631) 853-2299
Persons with an active case and medical need may get help with transportation to and from health care providers (doctor, clinic, hospital, etc.). All trips require prior approval. Call (631) 853-2299 between 8:30 a.m. and 4:00 p.m. for more information.
SPECIAL HEALTH CARE SERVICES
Individuals seeking information regarding non-institutional care such as personal care aide services or geriatric day care under the Medicaid Program should call Home Care Intake at (631) 854-9584 or 9585.
MA/SSI and MA/OVERAGE
SSI recipients needing Medicaid information should call (631) 854-5883. Persons who wish to "spend down" or pay an overage to qualify for Medicaid coverage, call (631) 854-5879.
MANAGED CARE: 1-888-566-9799
All ADC, HR and MA only clients, as well as some SSI clients as of April 1, 1998, may choose from seven managed care plans to receive the complete menu of Medicaid benefits. There are no costs, no caps, and no co-payments. Each member has his/her own physician or pediatrician-OB/GYN, who will coordinate all their health needs. Call New York Medicaid Choice at 1-888-566-9799 to receive a packet of informative brochures on each plan and an enrollment form.
