Admissions

If you or a loved one has been referred by a physician or a hospital, chances are that you qualify for skilled nursing care. The question then becomes one of choosing between the facilities available to you.

Selecting a skilled nursing facility is a very important decision, and many factors come into play. Is there high-quality nursing and medical care? What is the ratio of nurses to patients? Are the accommodations comfortable? Do the staff members seem friendly? Is the overall atmosphere a positive one? Do they welcome visits from family members? All these and more will contribute to an individual’s happiness once they become a resident.

Criteria for Acceptance
There are specific criteria that must be met in order for someone to be accepted as a resident at a skilled nursing facility such as John J. Foley. These include:

  • Medical needs that require skilled nursing care
  • A Patient Review Instrument (PRI) form completed by a certified RN
  • Pre-admissions screening and resident review form
  • A personal screening interview
  • A completed History and Physical form (when coming from the community rather than another health care facility)
  • A source of payment (we accept Medicare, Medicaid, and various private insurance plans)
