Admissions If you or a loved one has been referred by a physician or a hospital, chances are that you qualify for skilled nursing care. The question then becomes one of choosing between the facilities available to you. Selecting a skilled nursing facility is a very important decision, and many factors come into play. Is there high-quality nursing and medical care? What is the ratio of nurses to patients? Are the accommodations comfortable? Do the staff members seem friendly? Criteria for Acceptance There are specific criteria that must be met in order for someone to be accepted as a resident at a skilled nursing facility such as John J. Foley. These include: Medical needs that require skilled nursing care A Patient Review Instrument (PRI) form completed by a certified RN Pre-admissions screening and resident review form A personal screening interview A completed History and Physical form (when coming from the community rather than another health care facility) A source of payment (we accept Medicare, Medicaid, and various private insurance plans) There are also acceptance criteria for our Adult Day Health Care Program.
