Consumer Affairs continues to accept Occupational License, Renewal Applications and Complaint Forms.
Applications may be downloaded from our website, https://suffolkcountyny.gov/Departments/Consumer-Affairs/Applications-and-forms.
Fill in all information requested and return completed applications, and payment of fees, via mail, or secure Drop Box located at our Hauppauge County Center Office entrance.
Please call 631-853-4599 for assistance. A secured drop box is available at our Hauppauge location.

Make first right turn (North County Complex). We are the second building on the left, Building # 17, the entrance marked Consumer Affairs.
From East: Route 347 West and continue on Veterans Memorial Highway to Old Willets Path. Turn right on Old Willets Path and proceed as above.
