Department of Health Services Offices Set to Relocate
The Suffolk County Department of Health Services will relocate its administrative offices next week from 220 Rabro Drive and 225 Rabro Drive in Hauppauge to an office complex located in Great River. The move will be completed on Friday, August 31, 2012.
The following offices will move from 225 Rabro Drive: the Office of the Commissioner, the Office of Public Information, the Division of Public Health, the Division of Patient Care, the Office of Minority Health, the Offices of Finance and Administration, and the Office of Health Information Technology.
The following offices will move from 220 Rabro Drive: the Children’s Case Management Program, the Office of Compliance and Credentialing, the Immunization Action Program, the Bureau of Public Health Nursing (Western Suffolk), and the Office of Partner Notification and Disease Follow-Up.
All other divisions and offices within the Department of Health Services will remain at their current locations, and their phone numbers will remain current.

Residents who plan to visit the offices that are relocating during the week of August 27 to August 31, 2012 are advised to call the department’s current main number (631-853-3000) before planning their trip, or they may email the Department at: Suffolkcountydepartmentofhealth@suffolkcountyny.gov. Current email addresses will remain active.
For residents wishing to contact by phone the offices that are relocating, a listing of current and new phone numbers for each of the offices is provided below. The new phone numbers will be fully in effect on September 4th, 2012. Calls to 631-853-3000 after September 4, 2012 will be forwarded to the new main line.
