Applying for Medicaid Services

Effective January 1, 2014, most families and individuals must apply for health insurance, including Medicaid, through the New York State of Health program (also called the Health Exchange), either online, · by telephone or in-person through a Navigator or Certified Application Counselor. You can apply in the following ways:

The Consumer Assistance for the Aged, Blind, and Disabled program is overseen by the Suffolk County Department of Social Services, which subcontracts with the Nassau-Suffolk Hospital Council to provide free in­person application assistance at convenient locations throughout Suffolk and Nassau counties.
